Focus Care Agency is a Domiciliary Care Agency that offers individualised pathways of progressive care and support to people from their initial placements in hospitals, care homes or other settings by successfully reintegrating them into communities by providing holistic integrated supported living service or Housing with Care (HwC).
Our philosophy of service delivery aims to provide these high standard of comprehensive care/support that reflects an ethos of respect for people by treating them with dignity and ensuring that their right to privacy, safety, security and choice is maintained.
We enable/reable each person to maximise their independence by agreeing with them through information sharing and consultation, individual person-centred care/support plans, supporting them with risk-taking and encouraging the retention and development of daily life-skills. This is achieved by enlisting the support of the individuals family (by choice) other professional services and organisations that may be necessary from time to time and according to the defined needs of the service user.
Our Enhanced Care Recovery Approach (ECRA) is an approach to care that is holistic, integrated and person-centred. This comprehensive approach ensures that the pathway of care is service user focused and lead, outcomes based, tailored to meet the individual’s needs, expectations and perception of reablement/enablement, recovery and independence.
We offer individualised pathways of care and support to people from their initial placements in care homes, hospitals or low and medium secure settings by successfully reintegrating them into community settings by providing Supported Living Services and Housing with Care (HwC).
Our services are provided by a highly skilled and experienced multi-disciplinary team within facilities designed to meet the needs of our service users, which are modern and clean.
All support/care plans are agreed person-centred and encourage empowerment and involvement.
We work in partnership with the Service Users, their families, advocates and the multidisciplinary team to provide care and support that will enable the Service User to make informed choices and decisions in order that they may achieve their maximum potential for good health and well-being by living a quality of life and lifestyle as independently as possible within their local community.
Networking and close liaison with community teams (including if necessary Advocacy services, Mental Health & Learning Disabilities services, GP Surgeries, Community Nurses and Community Psychiatric Nurses) are maintained in the best interests of our service users. All services we deliver are transparent and flexible.
In addition, staff recruitment is rigorous and potential staff require a DBS (Disclosure and Barring Service) check, ISA first check, and at least two carefully cross-checked references. Staff must have a minimum of QCS Level 2 or equivalent alternatively working towards the qualification. All new staff are interviewed by Managers or Team leaders and where possible service users, before employment.
Staff training needs are constantly monitored and reviewed to ensure that the appropriate skill set are available to provide the care and support required by the service users i.e. Positive Behaviour Training; Makaton.

The social, emotional, spiritual and ethnic needs of our service users are met by maintaining and developing relationships with family, friends and social networks (Psycho-social awareness).
An on-going programme of improvement and facilities management is active to ensure service user environments are maintained to an excellent standard.
We are always ready to rise to the challenge in the provision of care and support to people with Complex needs – Learning Disabilities, Acquired Brain Injuries, Mental Health, Autistic Spectrum Disorders, Early onset Dementia and Behaviours that Challenge, which includes focusing on areas that are often not met elsewhere. We listen and respond to our customers through continuing consultation.
We constantly strive to improve our care and support delivery to meet the needs of our service users and to exceed the Essential Standards of Quality and Safety as set out by the CQC.
We acknowledge that reablement/enablement and recovery has a very personal definition for each individual; therefore our focus is based on service user lead support, hope, consultation, partnerships, self-direction, choice, respect, recovery, skills and resources.
